My name is Ash, I am a graduate of the University of Auckland Medical School and have recently begun further training in General Practice. I grew up in Whakatane and Tauranga and is a delight to return home and practice in the wider Bay of Plenty. In my spare time I can be found in the surf, on the ski fields or cycling in the forests.

Another miserable and wet evening last night. Staff from TNMC cooked and served a 3 course meal for Gina & Elmer and the people from Lovesoup. Everyone seemed to enjoy the meal, best of all was Jackie’s steamed pud, served with Maureen’s real custard (no packet stuff here) and Sarah’s whipped cream. Sue had made a big pot of pumpkin soup, Henri a pot of thick bacon & lentil broth and there was lots of home baking from the rest of the staff to finish off. Ron and Frances also helped on the night serving and making drinks. We will be doing it again.

Last week at our staff meeting we invited along Elmer and Gina Peiffer from LoveSoup to hear about the amazing work they are doing in our community. Gina and Elmer feed those who are rough sleeping and homeless, seven nights a week as well as providing an advocacy service, helping those in need find housing, work, education, counselling….the list is almost endless! The staff were blown away by what this couple manage to contribute. We asked, how can we help? The answer, money for petrol. We thought, we can do that!

How do we develop Rheumatic fever? Rheumatic fever can develop after a ‘strep throat’ – a throat infection caused by a Group A Streptococcus (GAS) bacteria. Most strep throats get better and don’t lead to rheumatic fever. However, in a small number of people an untreated strep throat leads to rheumatic fever one to five weeks after a sore throat.

How does Rheumatic affect us? Rheumatic Fever can cause the heart, joints, brain and skin to become inflamed and swollen. While the symptoms of rheumatic fever may disappear on their own, the inflammation can cause rheumatic heart disease, where there is scarring of the heart valves. People with rheumatic heart disease may need heart valve replacement surgery. Rheumatic heart disease can also cause premature death in adults.

Who is most at risk? In 2015, 98 people were admitted to hospital for the first time with rheumatic fever. This year in the Lakes area there have already been 4 new cases identified. Māori and Pacific children and young adults (aged 4–19 years) have the highest rates of rheumatic fever.

What can YOU do to prevent someone in your family developing Rheumatic Fever? Rheumatic fever can be prevented by prompt diagnosis of a strep throat infection and treatment with antibiotics. If your child or a child in your care complains of having a sore throat, bring them into the surgery to see one of our nurses and have a throat swab. If your child is under the age of 17 years, there is no charge for this service.

Last month there was a measles outbreak in the Waikato, with students at both Fairfield College and Morrinsville College reported as having contracted the disease. The outbreak now appears to have spread to the South Island after a teenager, who recently visited Hamilton, also contracted the disease.

Measles is highly contagious; however it is also easily preventable. Two doses of the measles vaccine, MMR, will provide protection for you and your family. Anyone, from babies through to adults, who are not fully immunised are at risk of contracting the virus. Adults born prior to 1969 are less at risk as they were probably exposed to measles as a child and therefore have a natural immunity. Those with low immune systems are at a higher risk.

The measles virus is easily spread through the air by breathing, sneezing and coughing. You will not necessarily be aware that you have contracted the disease until you feel sick, however you will be infectious for 5 days before and 5 day after the symptoms appear.

One in three cases will develop further complications, some of which require hospitalisation, the most severe cases can be life threatening.

Te Ngae Medical Centre recommends that you check to see if you and your family are fully vaccinated as this is the only way to protect you and those around you, from the disease.

Zika virus is a flavivirus, closely related to dengue. Although outbreaks of Zika virus have previously been reported in tropical Africa and Southeast Asia, there have been more recent cases reported in the Pacific Islands. The mosquitoes that are able to transmit Zika virus are not normally found in New Zealand, so you should only be concerned if travelling overseas. Zika virus infection is symptomatic in only about one of every five cases and when symptomatic the infection presents as an influenza-like syndrome (low-grader fever, headaches, myalgia etc).

There are however more concerns that pregnant women who become infected with Zika virus can transmit the disease to their unborn babies, with potentially serious consequences. Until more is known, the Ministry of Health recommends that women who are pregnant or plan to become pregnant in the near term consider delaying travel to areas with Zika present.

This advice was received by TNMC from the Immunistion Advisory Centre in response to the programme that screened on TV3’s 3D about parents who believe the death or ongoing disabilities of their daughters are due to the Gardasil® vaccine.

 

We understand that when illness strikes it is important to know what the cause is, and that parents need to find a reason for their child’s illness. Unfortunately sometimes we cannot find a cause or as health professionals give a reason.

There is absolutely no evidence that the Gardasil® vaccine has caused death, complex regional pain syndrome (CRPS), postural orthostatic tachycardia syndrome (POTS), or any other related fatigue syndromes. Just because these conditions occurred after vaccination does not mean that they were caused by the vaccine.

 

Key points on safety:

Safety studies now include over 1 million individuals

Useful responses to questions and concerns

1. It is important to recognise that these events occur in girls of this age who are not vaccinated as well as those who are. At this stage, no relationship between Gardasil® and these events has been found, despite extensive ongoing research.

2. What has been demonstrated, is the dramatic reduction in genital warts, pre-cancerous lesions and dysplasia in countires that have implemented this vaccine into their schedules.

3. The reality is that the symptoms of some of these events, for example CRPS and POTS, occur with many other conditions so diagnosis is often very difficult. It is estimated that every year about 30 women in every 100,000 between the ages of 10 and 19 years may develop one of these conditions.

4. In NZ, there are 180 new cases every year of POTS or CRPS in girls and women aged 10-19, both before thevaccine was introduced and after.

From July 1st 2015, all children under 13 are eligable for zero fees general practice visits at Te ngae Medical Centre.  The ‘zero fees’ scheme is part of the Government 2014 Budget and also includes children under 13 being exempt for the standard $5.00 pharmacy charge for each presecription.

What does the Scheme Cover?

The zero fees scheme applies to a standard visit to a GP or nurse at the child’s regular practice (where they are enrolled) and also applies to injuries covered by ACC. If you choose to attend a practice where you child is not enrolled, you may be charged a fee.

Fees for extended consultations, procedures and any additional services e.g. cetificates or equipment, willb e charged in the usual way.

Pharmacy Presription Charges

From July 1 2015, the $5 charge for each item of prescription medicine will be removed for children under-13, though pharmacies may charges for extra services such as medicines delivery or packaging. Fees for medicines that are not fully funded by Pharmac remain the same. Check with your pharmacist about any additional fees.

Influenza Vaccines are arriving later this year

 

The influenza vaccine for 2015 Southern Hemisphere influenza season includes two new strains based upon recommendations from the World Health Organization (WHO) on the strains most likely to spread and cause illness in people this season.

Delivery of the funded influenza vaccines will be later in 2015 than in previous years due to the change in strains and the complex manufacturing process which will take longer than usual.

 

Continuous supply of vaccine is expected to be available by April

Te Ngae Medical would like to introduce Dr Ron Vail who has joined our team of Doctors.  Ron, (another keen mountain biker) is originally from the USA  and has been working in Rotorua as a GP for the last 7 years, he is a welcome addition bringing a new breadth of experience. Ron’s consulting days are  Tuesday to Friday and he is enrolling new patients.  Please call in at the practice to collect an enrolment form and we can guide you through the necessary personal identification requirements.
